INITIAL CONSULTATION
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Your medical history will be reviewed and evaluated by the doctor and a treatment discussed. A second appointment will be scheduled to provide treatment.
You will be requested to provide the following information at the time of your consultation:
- Your referral slip from your dentist and any X-rays if applicable
- A list of medications you are currently taking
- If you have medical or dental insurance, bring the necessary insurance cards and information
- Please complete and bring with you the Patient Health History forms online, or when scheduling your appointment request that we mail the forms to you
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.
A preoperative consultation and examination is required for patients undergoing IV anesthesia for surgery. You will also need an adult to drive you home. Please have nothing to eat or drink 6 hours prior to your surgery if you are having IV sedation.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, a heart condition, artificial heart valves and joints, taking a blood thinner, etc.)
